TO BOOK AN APPOINTMENT
We will ask you to complete a registration form before an appointment is booked. This is to ensure we are fully informed about your symptoms and can offer the most appropriate pathway for your treatment. We will need the following information:
- Your full name, address, phone number and email
- Your date of birth
- A brief description of your symptoms and any medication/treatment you are currently on or have previously had
- Details of your GP
- If you have private medical insurance, we will need your membership details.
- We will also ask you for a GP referral letter which can be emailed directly from the GP surgery. If you do not have a GP referral, we will still be able to see you in clinic
We are fully compliant with the Data Protection Act 2018 and never pass your details on to third parties. Please refer to our Privacy and Cookies section for further information about how we use your data.
